Job Summary
This position provides in-home support to families, that promotes a positive parent-child relationship and interaction and provides case management/linkage to community resources to parents/caregivers. To support and advocate for the family to receive their entitlements and benefits, housing connects, etc.
Duties And Responsibilities
- Conduct field visits and escort families to various service providers; medical appointments, Public Assistance centers, Social Security, OPWDD, Medicaid offices and housing services.
- Conduct a home visit in compliance with ACS requirements in which the needs of the family and child safety are assessed.
- Advocate and assist families with connecting to community-based resources/ organizations and making referrals to facilitate the services needed to the families.
- Provide support services to families receiving services, which includes supportive casework focusing on services goals, child welfare laws, improving family relations ships, coping skills and parenting, etc.
- Participate in activities including initial intakes, contacts, and service plan meetings, etc.,
- Obtain collateral documents quarterly and/or annually as provided, from medical, mental health treatment, substance use and academic programs and other programs.
- Monitor and obtain information on the family’s medical needs, including compliance with periodic physicals and immunizations.
- Works with parents and provide education on improving parenting skills and home management skills.
- Participates in monthly Unit and Clinical Team Meetings.
- Completes and documents casework activities in tracking logs/excel sheets, case record PROMIS, CONNECTIONS and all other systems in a timely manner.
- Schedules and attends meetings and conferences as needed.
- Conduct community outreach and relationships with external providers.
- Conduct monthly chart reviews of at least 10 case records per month.
- Complete sections of the FASP assessment as required by ACS and OCFS
- Meets required weekly face-to-face contacts with all caretakers, parents, children and other adults outside the home as assigned.
- Participate, coordinate, and attend school visits, IEP meetings and other educational supports. Monitor and obtain information on the child/ren school placement and educational performance.
- Working with individuals, family and friend support networks, and health care and mental health professionals to put services in place.
- Attends all required training to enhance skills working with the high-risk population.
Skills, Knowledge, Abilities:
- Ability to use communication skills that successfully convey and obtain accurate and complete information in a respectful, caring manner.
- Demonstrates the ability to organize and coordinate work within schedule constraints and handle emergent requirements in a timely manner.
- Ability to communicate effectively with supervisors, co-workers, and other community partners
- Ability to conduct interviews with families to obtain and provide the information needed to carry out case management responsibilities
- Knowledge and ability to use critical thinking and analytical skills used to formulate with the family person achievable plans for implementation
- Ability to perform the essential functions of the job with or without reasonable accommodation.
- Collaboration and facilitation skills to work with the individual, families and services providers.
- Demonstrates cultural sensitivity and competence in work with families from various ethnic and cultural groups, social-economic status and lifestyles.
- HS Diploma or GED required
- Child welfare experience preferred.
- Basic computer literacy preferred.
- Due to the nature of the work environment, work hours will be based on the needs of program and may vary.
- Performs other duties/responsibilities as assigned within the scope of the
- position.
