About The Role
As a part-time Care Coordinator at Season (up to 28hrs/wk), you will serve as a key liaison between patients, clinicians, and care-related services to ensure comprehensive, person-centered care. You will play a critical role in assisting patients through every stage of their care journey, helping them access the resources they need, answering non-clinical questions, managing order fulfillment, and supporting overall patient satisfaction. This position requires excellent communication, organizational, and problem-solving skills, as well as a passion for improving healthcare outcomes.
You will collaborate with clinical experts, support new and existing patients, and help navigate their care experience, ensuring a seamless process from onboarding through ongoing care. As part of the Care Coordination team, you will contribute to refining internal processes, enhancing patient engagement, and providing outstanding customer support. This role reports to the Care Coordination Manager.

What You'll Do
- Support New Patient Onboarding: Assist patients in joining Season, including guiding them through the referral and registration processes.
- Patient Care Navigation: Help existing patients with all non-clinical aspects of their care, such as scheduling appointments, managing food orders, and collecting relevant health metrics.
- Order Fulfillment: Oversee and manage food orders, ensuring timely and accurate delivery in collaboration with team members.
- Customer Support: Respond to patient inquiries via phone, SMS, and email, delivering prompt, compassionate, and effective assistance.
- Team Collaboration: Work closely with clinicians and other Care Coordinators to ensure timely and high-quality support for all patients.
- Process Improvement: Identify opportunities to enhance care coordination and order fulfillment processes, contributing to the development of best practices.
- Advocacy: Advocate for patient needs across the organization, ensuring a high standard of patient-centered care.
- Contribute to the achievement of established clinical goals and objectives and adheres to department policies, procedures, quality standards and safety standards.
- Participate in meetings as appropriate.
- 2-4 years experience in an administrative nutrition/clinical setting
- Exceptional customer service skills and a passion for supporting diverse patient populations.
- Strong problem-solving abilities with a proactive approach to challenges.
- Highly organized with excellent attention to detail and the ability to manage multiple tasks in a fast-paced environment.
- Strong written and verbal communication skills, with experience in phone, SMS, and email support.
- Ability to collaborate across teams and build strong relationships with colleagues and patients.
- Proficiency in Google Workspace, Notion and Slack (or similar platforms).
- Eagerness to contribute to a growing organization and improve processes.
- Comfort with ambiguity

- The hourly rate/salary range for this role is $23.00 - 27.00/hr commensurate with experience.
