Nurse Practitioner - Geriatric Medicine

We are seeking a compassionate and skilled Geriatric Nurse Practitioner to join our team and provide comprehensive healthcare to our geriatric patients. Responsibilities

• Conducting thorough patient assessments and developing individualized care plans for geriatric patients.
• Diagnosing and treating acute and chronic illnesses common in the elderly population.
• Collaborating with physicians, nurses, and other healthcare professionals to ensure coordinated and effective care.
• Providing patient and family education on geriatric health issues, medication management, and lifestyle modifications.
• Conducting regular follow-up visits to monitor patient progress and make any necessary adjustments to treatment plans.
• Participating in multidisciplinary team meetings to discuss patient cases and contribute expertise in geriatric care.
  
  

Requirements

• Master's degree in Nursing with a specialization in gerontology or geriatric care.
• Current Nurse Practitioner license and certification in the state of practice.
• Proven experience working in geriatric healthcare, preferably in a primary care or long-term care setting.
• Knowledge of geriatric syndromes, medication management, and end-of-life care.
• Strong interpersonal skills and ability to empathize with elderly patients and their families.
