What are the responsibilities and job description for the Designated Coordinator position at Seasons Adult Foster Care?
PURPOSE:
This role as a Designated Coordinator (DC) is a key role within the company that provides case management services to assigned individuals that receive supported living services in a community residential setting. Designated Coordinators are responsible for the safety and wellbeing of the Person Served according to Seasons Adult Foster Care’s high standards in behavioral health while maintaining and upholding Minnesota Department of Human Services(MN DHS) 245D statutes.
This position will, in collaboration with the Designated Manager and other Designated Coordinators, provide oversight to multiple sites with persons served and will provide training and supervision to the Direct Support Professionals (DSP) that will be responsible for day to day support services. This position will work in collaboration with the program and administrative teams as part of the interdisciplinary team providing holistic behavioral and medical support services in a community residential setting.
RESPONSIBILITIES:
Essential job responsibilities include, but are not limited to:
- Lead, manage, and hold DSP’s accountable to carry out their daily tasks resulting in the improvement of the lives of persons served.
- Instruct DSP’s in the proper way to implement the Support Plan Addendums (SPA) and service outcomes for the persons served in our care.
- Study, absorb, and maintain a current understanding of MN Department of Human Services (DHS) Rule 245d.
- Execute compliance of MN DHS Rule 245d, person centered care and federal regulations in all areas of the company.
- Ensure person-centered planning and decision-making processes are followed.
- Facilitate regular team check-ins to discuss persons served progress and needs.
- Service Coordination: Develop, implement, and evaluate individualized service plans for persons served to ensure compliance with MN DHS Rule 245d statutes, including, but not limited to: SPA, IAPP, SMA, and outcomes.
- Design (as necessary), implement, and use data systems to measure effectiveness of services and supports.
- Provide supervision, support, and evaluation of activities that include:
- Oversight of the license holder’s responsibilities assigned in Person Served Support Plan (SP) and SPA.
- Evaluate the effectiveness of training, methodologies, and progress on the Person Served outcomes based on measurable and observable criteria.
- Provide program improvement for services provided.
- Serve as the main point of contact for person-served extended teams, including but not limited to: Pharmacy, Medical providers, Psychiatric providers, Case workers, Guardians, Family of the Person Served and the internal team.
- Attend all assigned Person Served meetings and external appointments.
- Address and follow up on all Person Served concerns.
- Assist with Person Served new admission planning and intake.
- Participate in a rotation of On-Call with other administration members to assist with after hours communications and some scheduling needs.
- Prepare and submit all required documentation or reporting in accordance with federal, state, and company guidelines.
- Review and maintain accurate records using the company's current charting systems; ensuring all required documents and data are present, readily available, and consistent.
- Participate in the orientation, mentorship, and training of new employees.
- Provide clear direction, appropriate instruction and assistance, and attainable expectations to direct support staff.
- Set and oversee day-to-day activities for assigned locations.
- Oversee and manage any crisis situations, including coaching staff, re-establishing rapport, and collaborating with the Designated Manager on incident debriefing.
- Manage and oversee the overall well being and progress of persons served collaborating with appropriate individuals in the company for improvement, concerns, and adjustments.
- Collaborate with the Designated Manager on behavioral concerns of the persons served.
- Collaborate with the Designated Manager on the effectiveness of SPA delivery, methodologies, and progress on the persons served outcomes based on measurable and observable criteria.
- Ensure outcomes are being carried out and achieved for persons served.
- Collaborate with the Designated Manager regarding medication updates.
- Review, document, and ensure accurate medication administration for persons served.
- Collaborate with the Designated Manager on medical concerns of the persons served.
- Intentional one-to-one interaction and observation of persons served to identify program improvement areas.
- Ensure nutritional therapy menu is provided, that balanced nutrition is offered and provided to persons served and staff.
- Ensure that all team members are adequately trained, adhering to company policy and procedure, as well as filing the proper documentation and follow up documentation required by state, federal, or company if the are staff not meeting these requirements or are in violation of these policies and procedures.
- Manage and oversee that the DSP team is following the financial planning and budget for the home and persons served (when appropriate).
- Maintain property (interior and exterior) with excellence in terms of cleanliness, safety, and licensing requirements and regulations.
- Oversee and ensure infection control protocols, safety protocols, standard operation procedures, and emergency procedures are followed.
- All other duties as assigned.

QUALIFICATIONS:
Minimum Education and Experience:
One of the following requirements minimum:
- A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or with significant and chronic behavioral health concerns;
- An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or with significant and chronic behavioral health concerns;
- A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or with significant and chronic behavioral health concerns;
- A minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or with significant and chronic behavioral health concerns under the supervision of a qualified staff person.
- Must have a valid Minnesota Driver’s License and clear motor vehicle report.
- Ability to pass a background check upon hire and periodically throughout the course of employment.
PREFERRED:
We would like to see experience in these areas:
- Registered Nurse, Licensed Practical Nurse or Designated Coordinator with case management experience, preferably with a focus on behavioral health.
- Proven history of supervising staff to complete projects; developing and leading staff.
- Experience working with medical software to record, document, and complete files for persons served; Experience with Therap a plus.
- Five plus years of professional experience in the mental health, medical, or human services fields required with at least three years at the supervisory level preferred.
- Clinical expertise in crisis intervention, skill in working with the mentally ill population, and with individuals undergoing acute episodes of psychiatric distress.

Work Environment:
The work environment involves working in residential homes and out among the community alongside individuals (persons served) who suffer from persistent chronic mental illness and have the potential to be verbally, emotionally, and physically aggressive toward themselves, each other, and staff. Staff will be required to complete training to read the persons served and fellow teammates body language to anticipate actions and work together using verbal and non-verbal communication to deescalate a person served that may be struggling to control their own behavior.
