We are seeking a highly skilled and compassionate Floor nurse, LPN to join our memory care community. As a Floor nurse, LPN, you will play a vital role in providing exceptional care and support to residents living with memory impairments. You will work closely with the Director of Nursing to ensure the highest standard of care is delivered and that all regulatory requirements are met. This position requires strong leadership, excellent clinical knowledge, and a genuine passion for working with individuals with memory care needs.
Responsibilities:
Resident Care: Provide high-quality nursing care to residents with memory impairments, including assistance with activities of daily living (ADLs), medication administration, wound care, and monitoring of vital signs. Observe and assess residents' physical and mental conditions, promptly reporting any changes or concerns to the appropriate medical personnel. Assist residents in maintaining their dignity and independence, fostering a safe and supportive environment.
Care Planning and Documentation: Collaborate with the interdisciplinary team to develop individualized care plans for residents, considering their specific needs and preferences.
Accurately document residents' health status, medication administration, and care provided in accordance with facility policies and regulatory guidelines. Regularly update care plans based on observations and changes in residents' conditions, ensuring effective communication of pertinent information to team members.
Medication Management: Administer medications to residents as prescribed, adhering to established protocols and ensuring accurate documentation of medication administration.
Monitor residents for medication effectiveness, side effects, and adverse reactions, promptly reporting concerns to the appropriate healthcare professionals.
Oversee medication storage, organization, and maintenance, ensuring compliance with safety standards and regulatory requirements.
Collaborative Teamwork:Work closely with physicians, nurse practitioners, and other healthcare professionals to coordinate and implement appropriate care interventions.
Communicate effectively with residents' families, addressing their concerns, providing updates on resident well-being, and offering support and guidance as needed.
Collaborate with other members of the healthcare team, such as caregivers, therapists, and social workers, to ensure comprehensive and holistic care for residents.
Compliance and Safety: Adhere to all facility policies, procedures, and regulatory requirements, including infection control protocols, resident rights, and confidentiality guidelines. Promote a safe and secure environment for residents and staff, identifying and promptly addressing any safety hazards or concerns. Stay updated on current best practices, advancements in memory care, and nursing techniques through ongoing education and professional development.

Pay: $24.00 - $28.00 per hour
