Key Responsibilities:
- Conduct comprehensive assessments of patients and families.
- Develop and update individualized care plans, administering medications and treatments as needed.
- Provide nursing care, health education, and emotional support to patients and families.
- Coordinate care with physicians and interdisciplinary teams.
- Participate in on-call duties and ensure seamless care continuity.
Qualifications:
- RN license required and hospice/community health experience preferred.
- Strong communication and case management skills.
- Ability to work independently and compassionately in patient homes.

Pay: $74,000.00 - $75,000.00 per year
