What are the responsibilities and job description for the Patient Navigator position at Seattle Reproductive Medicine?
SRM has twelve leading fellowship-trained Infertility and Reproductive Endocrinology specialists with expertise in infertility testing, treatment, male infertility (two on-site urologists), IVF, donor egg, donor sperm, artificial insemination, egg freezing etc. Additional services include acupuncture, counseling, infertility support group, genetics, tubal reversal, vasectomy reversal and fertility assessments for women concerned about reproductive aging. We currently have six locations in: Seattle, Tacoma, Bellevue, Kirkland, Everett and Spokane.
Summary:
We are looking for a Patient Navigator that will sit out of our Everett clinic. This will be a full-time, full-benefits, 40 hours a week position. Must be available Monday-Friday from 8:00am-4:30pm. Additionally, the position will involve weekend rotation, with the requirement of working 3 weekends and being on call for 2 weekends. Finally, the candidate will also be expected to work 1 holiday per year.
We are seeking a dedicated and compassionate individual to join our team as a Patient Navigator. As a Patient Navigator, you will play a crucial role in helping patients navigate the healthcare system, providing support, guidance, and resources to ensure they receive the best possible care.
Responsibilities:
- Collaborate with navigator assistants to ensure all new patients receive the introductory portal message, along with associated preconception and diagnostic testing handouts, and send appropriate consents and training modules through Engaged MD.
- Collaborate with navigator assistants to connect with all new patients to review the SRM Welcome and Preconception handouts, and to review next steps for diagnostic testing.
- Collaborate with navigator assistants to send a follow up message to all new patients 1-2 weeks after initial appointment with resources for emotional/wellness support during fertility treatment. Assist with scheduling and questions if planned appointments are not already scheduled.
- Review and abstract outside patient records, inputting lab values into the EMR, as needed, or send specific lists to navigator assistant/virtual assistant on clinical team to input in the EMR.
- Set up and maintain preconception and low-tech treatment plan checklists in the EMR and communicate with patients to discuss screening, process, and timeline expectations.
- Coordinate low-tech treatment plans (OI/IUI) based on the provider’s treatment plan and under the supervision of the registered nurse navigator, providing patients with next steps.
- Provide direction and guidance for patients ordering anonymous donor sperm for IUI cycles.
- Participate in cycle-specific visits and daily rounding by relaying and clarifying provider instructions regarding medication instruction (schedule, dosage, and administration) and follow up appointments for both low-tech (OI/IUI) and autologous ART cycles (with the exception of trigger/pre-op instructions).
- Accept/make phone calls regarding laboratory values and semen study results with appropriate explanation under the direction of the provider and/or registered nurse.
- Provide support with phone call and portal triage, answering questions as appropriate after consultation with nurse navigator or provider. Delegate patient questions to the registered nurses on the team that require nursing assessment and/or nursing education to maintain appropriate scope of practice.
- Establish a compassionate environment by providing emotional and psychological support to patients and patients’ families. Comfort by anticipating anxieties, and answer questions as permitted.
- Document patient care and instruction in patient EMR; responsible for accurate daily data entry into the clinical system.
- Manage medication samples and consignment medications, receiving, documenting stock and reconciling inventory as needed. Dispense to patients (for home use) with appropriate documentation.
- Coordinate with other departments on the patient’s behalf, to get questions answered and facilitate treatment plans in the most efficient way possible.
- Maintain a clean and safe working environment by complying with procedures, rules, and regulations, and calling for assistance from other health care support personnel when appropriate
Requirements:
- Passionate concern for our mission, vision, and values.
- Certified medical assistant with experience in women’s health, experience as a case manager in healthcare, and/or a bachelor’s degree in the sciences.
- Ability to work independently to manage a caseload of patients.
- Superior critical thinking skills.
- Ability to multi-task.
- Excellent computer skills with experience in Microsoft Office preferred.
- Ability to communicate accurately and concisely, with excellent interpersonal skills.
- Possess a strong understanding and appreciation of a diverse work environment.
