Job Description:

As a Home-Based Family Services Case Manager, you will play a crucial role in providing comprehensive support and assistance to families in need. You will work closely with families to assess their needs, develop individualized service plans, and connect them with resources and supports to address challenges and promote stability.

Home-Based Family Services Program:

Our Home-Based Family Services Program is a cornerstone of our commitment to accessible and effective mental health care. We recognize that families face unique stressors and dynamics that can impact their well-being, and we believe that providing therapy in the home environment can facilitate deeper connections and more meaningful progress. Our program is designed to:

  • Promote Family Well-Being: We work collaboratively with families to address a variety of concerns, including communication difficulties, conflict resolution, parenting challenges, and behavioral issues. By providing therapy in the home setting, we aim to strengthen family relationships and enhance overall well-being.
  • Tailor Interventions to Individual Needs: Our providers utilize evidence-based practices and personalized treatment approaches to meet the specific needs of each family. We recognize that every family is unique, and we strive to create individualized treatment plans that address the root causes of challenges and promote sustainable solutions.
  • Empower Families: We believe in empowering families to build upon their strengths and develop the skills needed to navigate life's challenges effectively. Through therapy sessions, skill-building exercises, and ongoing support, we empower families to take an active role in their own healing and growth.
  • Provide Culturally Responsive Care: We are committed to providing culturally responsive services that honor the diverse backgrounds and experiences of our clients. Qualifications:

  • Bachelor's or Master's degree in Social Work, Psychology, Counseling, or a related field.
  • Previous experience working with families in a social services or community-based setting preferred.
  • Strong assessment and case management skills, with the ability to develop and implement individualized service plans.
  • Knowledge of community resources and supports for families, including social services, mental health, housing, and education.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Commitment to cultural competency and providing culturally responsive services.
