RESPONSIBILITIES / DUTIES / FUNCTIONS / TASKS
Creates and maintains client case files on a daily basis.
- Performs intake and assessment of newly arrived Conditional and Eligible families referred from HERO (Housing Emergency Referral Office), Department of Homeless Services (DHS) within the 48-hour window as mandated.
- Ensure that the bulletin board in the security office has all the necessary photos and emergency contact information upon the arrival or discharge of every family in their case load.
- Escort client(s) to appointments (housing, entitlement, educational, social service, etc.)
- Ensure all records are available for review by the Program Director, Executive Director and the Case Manager Supervisor.
- Set up conferences with the Program Director and Case Manager Supervisor with clients who are non-compliant.
- Ensure those clients that are in the shelter 5 months, or more are seen on a weekly basis for an ILP (Independent Living Plan) review.
- Conduct weekly apartment inspections for clients with newborns 6 months or under and for families with ACS (Administration for Children Services) cases.
- Ensure clients submit copies of all their vital records, immunization records and physical forms at intake, etc.
- Ensure that corrections from Case Manager Supervisor caseload review are completed promptly and accurately.
