RESPONSIBILITIES / DUTIES / FUNCTIONS / TASKS

Creates and maintains client case files on a daily basis.

  • Performs intake and assessment of newly arrived Conditional and Eligible families referred from HERO (Housing Emergency Referral Office), Department of Homeless Services (DHS) within the 48-hour window as mandated.
  • Ensure that the bulletin board in the security office has all the necessary photos and emergency contact information upon the arrival or discharge of every family in their case load.
  • Escort client(s) to appointments (housing, entitlement, educational, social service, etc.)
  • Ensure all records are available for review by the Program Director, Executive Director and the Case Manager Supervisor.
  • Set up conferences with the Program Director and Case Manager Supervisor with clients who are non-compliant.
  • Ensure those clients that are in the shelter 5 months, or more are seen on a weekly basis for an ILP (Independent Living Plan) review.
  • Conduct weekly apartment inspections for clients with newborns 6 months or under and for families with ACS (Administration for Children Services) cases.
  • Ensure clients submit copies of all their vital records, immunization records and physical forms at intake, etc.
  • Ensure that corrections from Case Manager Supervisor caseload review are completed promptly and accurately.

  • Assist clients in obtaining entitlements, school enrollments and programs, adult training programs and support groups.
  • Refer and / or enroll families in substance abuse, family counseling rehabilitation programs, pre-natal programs, and any other medical and mental health programs available throughout the community.
  • Maintains follow-up communications between clientele and agencies on a daily basis.
  • Refer clients to housing specialist and assists in obtaining all the proper documentation pertinent for housing application completion.
  • Answer and follow-up on any incident reports of client violations within the facility from security reports and submit them to the Program Director and Case Manager Supervisor.
  • Report critical incidents to the Program Director and Case Manager Supervisor.
  • Attending scheduled training and workshops and present retrieved information to appropriate on-site staff.
  • REQUIRED QUALIFICATIONS

  • BSW or BA in Sociology or related field.
  • 3 years’ experience with the homeless population.
  • Thorough knowledge of social service networks.
  • Ability to diffuse confrontation.
  • Legible written and interpersonal communication skills.
  • Ability to work as a team, interconnecting with fellow staff on updated resources and clientele statuses.
  • Able to perform administrative duties as required through protocol.
