Job Summary:

Second Chance Case Management is seeking an experienced and motivated individual to join our team as a Care Coordinator/Social Worker. The Care Coordinator/ Social worker will perform work of moderate difficulty by providing skilled casework services to selected caseloads or clients with particular problems such as health disability or those at risk of nursing home placement; provides specialized casework services aimed at securing the client's overall well-being and maximum degree of independent functioning. Serves large geographic areas, including one large county and/or many small counties that may involve travel, and performs related work as required.

Duties/Responsibilities:

  • Understands the scope and purpose of the CCSP/EDWP Waiver Program
  • Identifies the client's needs and desired services
  • Researches and maintains up-to-date knowledge of community resources
  • Participates in case of conferences with the RN/LPN Case Management to discuss the care plan as needed.
  • Serves as the liaison between the assessment process and the effective delivery of direct services
  • Arranges for non- Traditional/Enhanced EDWP community-based services needed by the client
  • Notifies Operations of any change in client status.
  • Monitors service delivery to individual clients to ensure services are being provided as appropriate and effectively meet the client's needs
  • Continuously reviews, monitors, and updates the comprehensive care plan
  • Documents case activity and service information
  • Communicates and coordinates with all agencies providing direct services to the client
  • Monitors requests for increased services
  • Conducts Monthly Contacts via phone, email, or texts in consultation with the client/caregiver
  • Visits the client every 90 days to conduct a Care Plan Review
  • Reports suspected abuse, neglect, or exploitation of any client to APS
  • Arranges emergency services
  • Maintains confidential case records on all Traditional/ Enhanced EDWP clients
  • Maintains knowledge of the provider service standards for each Traditional/ Enhanced EDWP service
  • Performs other duties as assigned
  • Meets with the supervisor at least monthly to discuss and review cases
  • Attends organizational meetings and training as required
  • Keeps supervisor informed of progress and problems associated with duties

Required Skills/Abilities:

  • Ability to effectively coordinate and communicate
  • Skill in establishing and sustaining interpersonal relationships
  • Knowledge of community organization and service system development
  • Problem-solving skills and techniques
  • Knowledge and skill in social and health service intervention techniques and methodology.

Education and Experience:

  • Bachelor's degree in social work, sociology, psychology, or a related field

Pay: From $50,000.00 per year
