Qualified Mental Health Professional – Community Services (QMHP-CS)

Job Description

Job Objective:

Provide Mental Health Targeted Case Management (MHTCM) and Mental Health (MH) Rehabilitative services to individuals with a diagnosis or diagnoses of mental illness or serious emotional disturbance (SED) as defined in the latest edition of the American Psychiatric Association’s (APA) Diagnostic and Statistical Manual of Mental Disorders (DSM).

QMHP-CS are expected to model through their behaviors and attitudes the values of Second Chances Counseling and Wellness as outlined in the Policy and Procedures Manual. Services are interventions provided in the community and/or in the home of the client who needs assistance in maintaining or improving functioning due to APA DSM diagnosis or diagnoses.

Immediate Supervisor: Licensed Practitioner of the Healing Arts (LPHA)

Primary Responsibilities:

Clinical Services are primarily to be provided via face-to-face involvement with the client and other family members primarily in the community and/or in the client’s home. The QMHP-CS is required to assist in the development and implementation of the comprehensive assessment; and a person-center, recovery-based treatment plan. The activities associated with the primary responsibility of this QMHP-CS are:

  • Ongoing assessment of treatment needs.
  • Assist in the development and ongoing monitoring and implementation of Individual Service Plan (ISP).
  • Work with clients to foster the development of new improved coping, problem solving, and communication skills.
  • Maintain continuous contact with the identified consumer and other key stakeholders (with signed authorization to release information) in planning and service delivery. These stakeholders might include but are not limited to social workers, psychiatrists, juvenile court officials and police, teachers and other human service professionals
  • Educate family members regarding existing community services, and act as an advocate in connecting family members to these services
  • Respond to individual in crisis on a 24 hours, seven day a week basis
  • Maintain up-to-date client records, and prepare required/necessary reports in a timely and accurate manner
  • Establish a good working relationship with other public and private agencies/services and interface collaboratively with them

The QMHP-CS is required to provide service coordination activities for the ISP development and implementation as such they must have the following knowledge skills and abilities:

  • Ability to provide and/or deliver various skill building activities
  • Knowledge and experience with crisis situations and interventions
  • Ability and skills for enhancing caregiver roles
  • Ability to assist consumers in developing daily and community living skills
  • Ability to assist consumers in development of leisure time interests/ activities
  • Knowledge, skills, and ability for medication management, symptom monitoring and management
  • Ability to assist consumers in developing behavior and anger management skills
  • Ability and skills to maintain required confidentiality regarding client case materials in compliance with the Health Insurance Portability and Accountability Act (HIPAA)
  • Knowledge skills and abilities to provide closure and aftercare planning for each client/family
  • Ability and skills to work with children, adolescents, and/or adults who have serious emotional disturbances and who may be hostile, oppositional and volatile, while at all times maintain composures and behave in a dignified manner.
  • Knowledge about normal adolescent development
  • Knowledge about abnormal adolescent behavior
  • Knowledge about adolescent and adult maladaptive behaviors
  • Knowledge and a firm understanding of family developmental issues
  • Skills and abilities necessary to develop/write the required direct or collateral/stakeholder contacts in progress note format.

Supervision:

  • Participate in individual and team supervision as scheduled.
  • Experience with supervising Community Services Specialists (CSSP) preferred but not required

Academic/Work Experience:

  • Bachelor’s or Master’s in Human Services field. Candidate must qualify as a QMHP-CS as defined in the Texas Medicaid Health Provider (TMHP) regulations.

Training:

  • Staff must be trained certified in: Cardio Pulmonary Resuscitation (CPR), First Aid, Management of Aggressive Behavior or Behavioral Supports.

Additional Requirements:

  • Employee must be certified as being tuberculosis free
  • Employee must clear criminal background and CPS registry checks
  • Employee must have good driving record and proof of active car insurance

Job Types: Full-time, Contract
