Position: Care Manager
Salary: $46,500/year, plus healthcare coverage (dental, vision), paid holiday/vacation/sick time, and mileage stipend. After probationary period, earn up to additional 1250.00 every quarter in bonuses based on performance reviews. Health benefits (medical, dental, vision, short-term disability) begin after a 60-day probationary period. Total: 51,000 annually.
Work Schedule:
Full-time, exempt. 5-day work weeks, with rotating weekend schedules. Occasional after-hours work may be required for emergencies. Mandatory attendance at fundraising events when scheduled, approximately 3-5 a year.
Role Purpose and Key Results:
As a Care Manager, your primary responsibility will be to achieve measurable improvements in the lives of individuals with a mental health diagnosis and/or substance abuse history by providing ongoing support and resources. You will focus on empowering residents to meet their personal goals, maintain safe and healthy living environments, and build sustainable life skills. You will contribute directly to our mission of transforming lives through compassionate care and innovative solutions.
Your success in this role will be measured by:
· Resident progress: Measurable outcomes in resident goals, such as improvements in personal care, employment, turnover rate and mental health stability.
· Service coordination: Effective collaboration with community partners and agencies to ensure resident receive the services and support there need.
· Resident engagement: High levels of resident engagement and satisfaction with tailored services.
· Operational efficiency: Timely completion of administrative tasks and adherence to program guidelines.
Key Responsibilities:
· Resident Empowerment: Conduct regular daily visits to SHH homes to monitor resident well-being, assess needs, and help resident set and achieve realistic, attainable goals. Track and measure progress to ensure outcomes are met.
· Move-Ins and Relocations: Coordinate resident transitions, including move-ins, relocations, and discharges. Ensure homes are safe and compliant with cleanliness standards.
· Service Coordination: Act as a central point of contact for external agencies and community resources, facilitating access to public benefits, healthcare, and other services based on the resident care plan.
· Crisis Management: Resolve emergencies and support resident in crisis situations, ensuring their safety and well-being while maintaining compliance with all policies.
· Resident Engagement: Build strong, trust-based relationships with resident, fostering open communication to resolve issues and support their development.
· Case Documentation: Maintain up-to-date, accurate resident records and report progress through our case management system in compliance with professional standards. Attend team staffing and submit a monthly report on your resident’s progress.
· Workshops & Life Skills: Develop and lead workshops designed to build life skills, empowering resident to improve their quality of life.

Qualifications and Experience
· Bachelor’s degree or relevant experience in a social services setting required.
· 2 years working with homeless, substance abuse history and mental diagnosed individuals.
· Training or experience in detecting when a person is manic or having a crisis moment/episode. Able to detect red flags!
· CPR certification preferred and knowledge of local community resources required.
· Proficiency in Microsoft Office (Word, Excel, Calendar)
· Reliable transportation and maintain a valid driver’s license and insurance.
· Ability to work flexible hours, including occasional after-hours emergencies.
· Ability to lift up to 30 pounds and perform physical tasks related to resident services (such as moving bags of clothing or moving a box).
· Work in a diverse setting, including outdoor environments and resident homes.
· Ability to self-reflect, maintain biases and maneuver in a stressful situation.
