Role:The Member Engagement Coordinator handles welcoming the Members to the CareNu community and providing entry levels assessment to ensure the appropriate Care Manager is assigned. Works in conjunction with the Member, family members and Care Managers across multiple platforms to educate on the benefits of the assigned model while increasing buy-in and adherence. Ensures a smooth handoff is conducted between departments and all initial concerns are rectified. Leads customer engagement surveys to monitor for excellence among all product lines.
Qualifications:
- Bachelor’s degree in Human Services or related field preferred
- Minimum of one (1) year of healthcare related roles
- Solid communication (verbal and written) skills
- Highly organized and self-motivated
- Must understand basic functions of the care continuum and have a solid record of working with patients and families through various mediums
- Bilingual in Spanish preferred

Job Responsibilities:
- Provides initial welcome call to Members and families letting them know they are part of the network.
- Evaluates education needs and describes what an Accountable Care Organization (ACO) is and how they were selected.
- Educates on benefits and resources available.
- Develops and maintains strong knowledge base of high needs resources and how CareNu works to get them connected within the community.
- Earns mutual trust with families and Members to build rapport.
- Conducts social determinists of health surveys for all new Members and then, quarterly.
- Ensures smooth handoffs of Members from the Engagement Department to Care Managers.
- Performs other duties as assigned.
Compensation Pay Range:
$19.10 - $27.51This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.
