• Assesses needs and coordinates resources within and outside the benefit plan (as approved by the CMO/Medical Director) to promote optimal health benefits and outcomes• Onsite inpatient case review at specific hospitals and other facilities as assigned
• Establishes rapport with local hospital case management, hospitalist, ER case management and discharge planning staff to expedite timely transitions of care and discharge of SECUR members
• Collaborates with physicians, acute/sub-acute case management staff, members and deemed member representatives to stay within the benefit plan, identify member needs/resources and discharge requirements
• Facilitates problem solving and decision making by providing guidance, plan benefit knowledge, knowledge of disease management systems, wellness resources and supportive services and options to the member, member representative and health care providers
• Provides direct delivery of health management services as determined by member/population need, including but not limited to, specific program referrals such as CM/DM and education
• Establishes the most appropriate plan for continued stay and discharge with the direction of CMO and Health Services Director
• Functions as a key member of the interdisciplinary team
• Performs other duties as assigned
Qualifications:
- Current RN Florida license, current active single-state out-of-state RN license, or current active multistate/compact RN license
- Minimum 5 years of clinical, utilization and management experience
- Advanced certification in either Care Management or Certified Professional in Health Care Quality (CPHC) preferred
- Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
- Proficient computer skills including Excel
- Ability to convey empathy, compassion and caring
- Ability to effectively communicate in English (verbal and written)
- Mobile Driver - Valid driver’s license and automobile insurance per Company policy

Compensation Pay Range:
$64,074.00 - $96,111.00This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.
Salary : $64,074 - $96,111
