Community Health Worker

Job Summary
The Community Health Worker (CHW) serves as a vital link between healthcare providers and the community, working to improve health outcomes and promote wellness among diverse populations. This role involves engaging with individuals and families to provide education, support, and resources related to public health initiatives. The CHW will utilize their knowledge of medical terminology and public health practices to address various health-related issues, particularly for individuals with disabilities and those navigating Medicare services.

Duties

• Conduct outreach and education to community members regarding public health resources and services.
• Assist individuals in understanding their health conditions and navigating the healthcare system.
• Utilize motivational techniques to encourage healthy behaviors and lifestyle changes.
• Collect and analyze data to identify community health trends and needs.
• Collaborate with healthcare professionals, social workers, and others to coordinate care for clients.
• Provide support for individuals with disabilities, ensuring they have access to necessary services and accommodations.
• Facilitate workshops or presentations on relevant health topics, employing effective public speaking skills.
• Maintain accurate records of interactions with clients and report on outcomes as required.

Requirements

• Knowledge of public health principles
• CHW Certification
• Strong communication skills with the ability to engage diverse populations effectively.
• Experience working with individuals with disabilities is highly desirable.
• Proficiency in data collection methods for assessing community health needs.
• Ability to work independently as well as part of a multidisciplinary team.

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
