We are seeking a compassionate and dedicated Registered Nurse - Home Health to join our team. Responsibilities
- Conduct comprehensive assessments of patients' health status and needs.
- Develop and implement personalized care plans based on patient evaluations.
- Administer medications and treatments as prescribed, including acute care procedures.
- Provide education to patients and families regarding health management, medication administration, and care techniques.
- Perform medical documentation accurately and timely using eClinicalWorks or similar systems.
- Assist with specialized procedures such as gastric lavage and botulinum toxin treatment when necessary.
- Offer support in dementia care, focusing on enhancing the quality of life for patients with cognitive impairments.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Maintain a safe and sterile environment while adhering to infection control protocols.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in home health care or similar settings preferred.
- Proficiency in medical documentation practices.
- Knowledge of primary care principles and acute care nursing practices.
- Familiarity with sterile processing techniques is a plus.
- Strong communication skills to effectively interact with patients, families, and healthcare teams.
- Ability to work independently while managing multiple patient cases effectively.
