RN Night Shift Nursing Home

Overview
We are seeking a compassionate and dedicated Registered Nurse - Home Health to join our team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will work closely with patients and their families to develop individualized care plans, monitor health conditions, and administer treatments as needed. Your expertise in various nursing practices will be essential in delivering exceptional care to those who require assistance in a home setting.

Responsibilities

• Conduct comprehensive assessments of patients' health status and needs.
• Develop and implement personalized care plans based on patient evaluations.
• Administer medications and treatments as prescribed, including acute care procedures.
• Provide education to patients and families regarding health management, medication administration, and care techniques.
• Perform medical documentation accurately and timely using eClinicalWorks or similar systems.
• Assist with specialized procedures such as gastric lavage and botulinum toxin treatment when necessary.
• Offer support in dementia care, focusing on enhancing the quality of life for patients with cognitive impairments.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Maintain a safe and sterile environment while adhering to infection control protocols.

Qualifications

• Current Registered Nurse (RN) license in the state of practice.
• Experience in home health care or similar settings preferred.
• Proficiency in medical documentation practices.
• Knowledge of primary care principles and acute care nursing practices.
• Familiarity with sterile processing techniques is a plus.
• Strong communication skills to effectively interact with patients, families, and healthcare teams.
• Ability to work independently while managing multiple patient cases effectively.
• Compassionate approach towards patient care with a focus on enhancing their overall well-being.

Pay: From $33.00 per hour

Expected hours: No less than 36 per week
