What are the responsibilities and job description for the CCBHC Patient Navigator - COMCARE position at Sedgwick County?

Pay : $23.46 per hour | This position may also be eligible for a sign on bonus of $2,500 after 60 days of employment if selected applicant meets all policy conditions.
Work Schedule : Varied schedule, Monday-Friday between 7 : 00am - 7 : 00pm

Provide services and support to patients who are involved in COMCARE’s Certified Community Behavioral Health Clinic (CCBHC) to improve the quality of life and ability to function more successfully in the community. Computer documentation of all services in a timely manner is required. Navigator will work to collect and enter data into the SPARS reporting system.
Complete and collect National Outcome Measures (NOMS) and other relevant data
- Work with patients to gather and complete NOMS questionnaires.
- Enter NOMS data into the SPARS system in a timely manner.
- Complete required patient documentation at intake.
- Monitors assigned CCBHC outcomes and coordinates with internal and external providers to increase frequency of desired outcomes.
- Utilizes outcome tracking to help achieve successful outcomes and approve quality of care provided.
- Collaborates with other team members regularly.

Minimum Qualifications : Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. If no degree is present applicant may substitute each one year of experience for one year of education. Must have a valid Kansas Driver’s license and current proof of automobile insurance. Must be able to safely operate a motor vehicle and meet driver qualification standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage policy. Meet the specifications as outlined in the CMHC / CCBHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens. Must complete orientations provided by Sedgwick County and COMCARE.
Preferred Qualifications : One year of related experience.
