ABOUT THE TEAM ????
The Supported Living (SL) team provides support for a variety of men and women, within a time 24 hours a day time frame, seven days a week if needed. Supports range from health, safety, and community participation for those with more significant needs. The team also provide services for persons in his/her home to include health, safety, and community participation as well as assisting in coordinating the activities of the home by eliciting the preferences and needs of and providing support to persons.
This position will provide support and assistance for an adult with developmental disabilities. This position is located in Germantown, MD for every other weekend, Saturday and Sunday (9am - 9pm).
IN THIS ROLE, YOU'LL: ????
Person-Centered Approach Support
- Communicate status and actively participate in person and team/caseload
- Demonstrate respect and acceptance of persons and their
- Proactively implement systems to ensure quality services for persons in designated areas such as health, safety, and community participation.
Personal Care
- Provide personal care to include eating, using the bathroom, bathing/showering, transferring, shaving and personal hygiene issues.
- Conduct quality service checks to include conditions of home, personal care, adequate household, and personal supplies.
Community Engagement/Inclusion
- Research and communicate information regarding community resources and other resources (i.e.: education, spiritual, leisure, transportation and medical).
- Assist persons and families through the access and eligibility process for public transportation and Para
- Facilitate successful person participation in events, including staff training, community education technology and resource development.
Medical
- Administer medications as delegated by delegating nurse. Pick up medications as needed.
- Track, book, and accompany persons to medical appointments, ensuring that required medical documentation is completed.
Household
- Review and sign off on all household purchases delegated to support.
- Handle ATM card to include coordinating, storing, and submitting receipts, verifying.
- Monitor and follow-up on household maintenance and other designated issues, reporting issues to Support Team Coordinator as required.
Collaboration Across Teams
- Collaborate with Support Team Coordinators and staff to ensure person participation such as transportation, accommodations, etc.
- Exhibit professionalism as well as exercise sound judgment in personal presentation and interaction with persons, co-workers, employers, and colleagues.
- Create on-going, positive relationships with community business/organizations (including transportation services, schools, recreation, and entertainment business, etc.).
Meetings
- Attend scheduled meetings and trainings, internal and external, to the agency.
- Complete and ensure staff completion of mandatory documentation as required, such as Personal Vision Plan activities and behavior program data.
Other
- Perform other job duties as assigned
ABOUT YOU ????
- High School Diploma Preferred.
- 2 years’ experience in supporting people to achieve personal goals, or experience of a comparable nature that has helped candidate build community-engagement skills.
- Strong analytical skills, good mentoring and coaching abilities and professional approach to personal development.
- Must be willing to learn and use technology, such as cell phones, iPhone apps, photo, videos, laptops, adaptive technology, and case management database.
- Must possess an excellent performance ethic and be a strong team player.
- The ability to lift 60-80 lbs.
- Must have knowledge and familiarity of the surrounding counties.
- A valid Driver’s License.
