What are the responsibilities and job description for the Adult Nurse Practitioner- Gulfport, MS position at Segue Health Management Corp?
Segue Health, a faith-based medical practice, is looking to hire a Nurse Practitioner to work in Gulfport, Ocean Springs, Biloxi and surrounding areas to see patients for in-home transitional care management and treatment after discharge from an acute care facility. The candidate will provide medical consults, review discharge summaries, review progress notes, collaborate with home health or other members of the healthcare team and prescribe medications when necessary.
Must have the ability to travel up to 1 hour away from home. Candidates needed to cover the following areas: Gulfport, Ocean Springs, Biloxi and surrounding communities. Candidates must be able to provide dedicated day(s) each week for seeing patients.
Must have a current medical license to treat within the state of MS and a current Medicare number.
