About the Role:
- Provide high-quality patient care to chronically and critically ill patients, utilizing evidence-based practices.
- Collaborate with healthcare teams to develop and implement individualized patient care plans, addressing complex medical and psychosocial needs.
- Ensure accurate and timely documentation, using industry-standard electronic health records.
Key Responsibilities:
- Initial and ongoing patient assessment, identifying needs and developing plans to address them.
- Interpret diagnostic data, lab results, and other relevant information to inform patient care decisions.
- Communicate effectively with patients, families, and interdisciplinary teams to ensure seamless transitions of care.
Qualifications:
- Current State RN License and BLS certification required.
- ACLS certification within six months of hire.
- Experience working with critically ill patients and their families highly preferred.
