We are seeking an enthusiastic, reliable, and motivated Case Manager for our Coordinated Family Support (CFS) program to join our team. This role involves providing crisis management, conducting assessments, developing service plans, coordinating and monitoring service delivery, and advocating for clients' rights and access to services. The position requires about 32% travel within the Los Angeles area.
Responsibilities
- Provide crisis management and intervention services.
- Conduct assessments to determine client needs and develop appropriate service plans.
- Coordinate and monitor the delivery of services.
- Collaborate with other professionals, agencies, and community resources.
- Utilize motivational interviewing techniques to engage clients and promote positive change.
- Maintain accurate and up-to-date client records and documentation.
- Participate in case conferences and team meetings.
- Advocate for clients' rights and access to services.
- Significant local travel to meet with clients and coordinate services.
Requirements
- Strong communication, organizational, and interpersonal skills.
- Ability to work effectively both independently and as a team member.
- Proficiency in both Spanish and English REQUIRED.
- Valid driver's license and access to a reliable and insured vehicle.
- Experience with Google Workspace
- Bachelor's degree in social work, psychology, or a related field (preferred).
