Overview
We are seeking a dedicated and organized Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that patients receive the highest level of care by facilitating communication between healthcare providers, patients, and their families. This position requires a strong understanding of medical terminology and administrative processes within a medical or dental office setting.
Location: Hybrid (50% work from home, 50% field visits within designated areas in King and Snohomish County)
Pay: Pay per member served
Expected Hours: Part-time.
Duties:
- Schedule and coordinate patient appointments to maximize provider efficiency.
- Maintain accurate, up-to-date patient records.
- Communicate appointment reminders, follow-up care, and test results to patients.
- Assist with insurance verification and pre-authorization.
- Collaborate with healthcare providers for seamless care coordination.
- Provide administrative support to the healthcare team.
- Organize and facilitate care activities for a holistic health approach.
- Engage clients through community-based, face-to-face encounters and in-home visits.
- Conduct comprehensive health screenings and develop individualized Health Action Plans.
- Support clients in goal setting, care planning, and managing their own health.
- Promote client engagement in work, volunteer activities, and social support networks.
- Assist with mental health and substance use treatment as needed.
Travel Requirements:
- Travel to client/patient homes within designated areas.
- Access to reliable transportation with mileage reimbursement provided.
- Valid Washington State Driver’s License required.
Experience & Qualifications:
- Care Coordinator with previous experience in Health Home program highly preferred.
- Bachelor’s degree in healthcare, behavioral science, social services, or related field; LPN, Peer counsellors. Certified Medical Assistants with an associate degree
- Familiarity with medical terminology and electronic medical record (EMR) systems.
- Strong organizational, communication, and interpersonal skills.
- Proficiency in MS Office (Word, Excel, Outlook).
- Ability to multitask and work in a fast-paced environment.
- Demonstrated knowledge or interest in behavioral and physical health issues.
- Current First Aid & CPR certification or ability to obtain within 90 days of hire.
- Bilingual/bicultural skills are encouraged.
- Experience in social work or related fields is preferred.
- SSN and Work permit required.
