The Perinatal Navigator (CHW) at Self Regional Healthcare is dedicated to providing crucial support to pregnant individuals and new mothers. Collaborating with a multidisciplinary team, the CHW will ensure that perinatal care is comprehensive and coordinated, focusing on establishing trusting relationships, providing health education, and connecting clients with essential resources.
Essential Job Elements:
1. Establishing Trusting Relationships:
• Build and maintain trusting relationships with pregnant individuals and new mothers.
• Provide ongoing support, encouragement, motivational interviewing, and goal setting.
• Conduct follow-ups via phone calls, home visits, and visits to other settings where clients can be found.
2. Collaboration with Care Team:
• Work with the healthcare team to assist clients in setting personal goals, obtaining prenatal and postpartum care, and attending appointments.
• Help clients utilize resources, schedule appointments, and complete necessary paperwork.
• Act as an advocate and liaison between clients and community service agencies.
3. Health Coaching and Advocacy:
• Coach clients in the effective management of chronic health conditions and self-care during pregnancy and postpartum.
• Assist clients in understanding care plans and instructions.
• Collaborate closely with medical providers and other care team members to ensure comprehensive and coordinated care.
4. Referrals and Coordination:
• Provide referrals for services to community agencies as appropriate.
• Facilitate communication and coordinate services between providers.
• Motivate clients to be active, engaged participants in their perinatal health.
5. Environment of Collaboration and Teamwork:
• Foster an environment of collaboration and teamwork that promotes an outstanding perinatal experience.
• Coordinate with primary care, OBGYN, and other departments and community partners.
6. Case Load Management and Documentation:
• Manage a case load of perinatal clients.
• Document all interactions with clients and care providers in the approved software system, including appointments, phone calls, home visits, and written correspondence.
7. Operations Support:
• Support all operations initiatives implemented by organizational leadership with a positive attitude.
• Offer constructive feedback and recommend solutions when processes are not operating optimally.
• Provide feedback to leadership on potential improvement opportunities.

• Associates Degree in other fields with 5 years of experience in healthcare, care coordination, or case management will be considered.
