Job Requirements:
- Bachelor's Degree in a Human Service field required
- West Virginia Social Work License preferred
- Demonstrated knowledge of basic computer skills required
Key Responsibilities:
- Advocate on behalf of clients to ensure continuity of services and accessibility to services
- Ensure adequate crisis response planning procedures are available to clients and assist as necessary in accessing crisis support services and interventions
- Coordinate warm hand-offs with other staff to ensure clients receive recommended treatment services
- Participate in Interdisciplinary Team Meetings, daily staffing, physician rounds, and discharge planning as needed
- Assure quality service for clients by adhering to rules, regulations, and legal requirements and documenting services provided in accordance with established standards
- Secure client information by adhering to HIPAA and 42 CFR Part 2 regulations and Seneca privacy policies
