Summary
The Physical Therapist is responsible for working with patients in the acute, swing, skilled nursing and outpatient settings as prescribed and ordered by the attending physician. They will evaluate, develop, and implement effective rehabilitation plans to improve patients' mobility, strength, balance, coordination, and quality of life. The Physical Therapist plays a key role in restoring patients' functional abilities after injury, surgery, or illness, ensuring their recovery process is comprehensive and individualized.
**Position includes a $10,000 one time bonus at the anniversary of the first year and up to $8,000 for relocation, receipts required.
Essential Functions
Inpatient Care Duties
- Assess and evaluate patients' physical conditions and functional limitations.
- Develop and implement individualized rehabilitation plans based on clinical findings and patient goals.
- Provide physical therapy treatments to inpatients recovering from surgeries, illnesses, or injuries.
- Collaborate with the multidisciplinary healthcare team, including physicians, nurses, and occupational therapists, to ensure optimal patient outcomes.
- Monitor patient progress and modify treatment plans as necessary.
- Educate patients and their families about the rehabilitation process and ways to prevent further injury.
Outpatient Care Duties
- Perform comprehensive evaluations of outpatient clients, assessing musculoskeletal, neurological, and other impairments.
- Design and implement therapeutic exercises and interventions to promote recovery and prevent re-injury.
- Work with patients on a one-on-one basis to restore functional mobility and manage chronic conditions.
- Conduct follow-up assessments to track patient progress and adjust treatment plans accordingly.
- Provide education on home exercises, body mechanics, and injury prevention.
- Performs other duties as assigned

Qualifications
- One year experience in acute care, extended care, rehabilitation, or outpatient setting
- Current Physical Therapy license from the State of California
- Current BLS certification (AHA)
