As an award winning perfessional in home care agency servicing senors in Edmond and Oklahoma City, Senior Care At Home is seeking a full-time Experienced Care Coordinator. This position is responsible for overseeing and coordinating care services with clients and care staff and ensuring high quality service. The Care Coordinator will have some clinical patient care experience and knowledge of medical conditions and terminology, a strong customer service focus, excellent time management and prioritization skills, and be proficient with computer systems and technology.
Responsibilities :
Conduct care consultations with prospective clients alongside RN staff, addressing their needs, answering questions, and building relationships with clients and families.Coordinate and oversee care services, including caregiver selection, scheduling, care plan reviews, caregiver orientation, and supervision.Provide ongoing care management, ensuring service quality, client and caregiver satisfaction, and handling scheduling changes and updates.Monitor and update care plans based on client needs, communicate changes, conduct home visits, and collaborate with RN staff for reassessments.Manage administrative duties, including documentation, policy communication, caregiver training coordination, and participation in company meetings.Maintain office hours, provide after-hours / on-call support as needed, and represent the company professionally in the community.Job Requirements :
Healthcare experience required. Experience in patient care coordination preferred; senior (geriatric) care preferred. LPN or therapist (PT / OT) license strongly considered.Knowledge of medical conditions and terminology, particularly for older adultsAvailable for work some weekends and eveningsProficient with using software, mobile technology, data entry, and correspondence.Ability to interface effectively and professionally with client stakeholders including family, healthcare providers, attorneys, etc.Ability to function in a small team environment as well as autonomouslyExcellent written and verbal communication skillsTravel in OKC metro area required.
