The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. Our mission is to empower independence and autonomy for participants and their support systems through exceptional interdisciplinary and integrated geriatric care. PACE is an alternative to nursing home care and through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides, we coordinate and provide preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.
Centralized Triage Nurse
Primary Purpose
Assures that participants will be assessed and triaged using the nursing process and approved guidelines to assist in obtaining the appropriate level of care and/or self-care advice. Able to remain independent in the application of advanced nursing knowledge and skills with the ability to manage complex clinical situations and facilitate appropriate routing of concerns.

- Perform assessments in the clinical setting and/or home/facility
- Effectively communicates treatments, procedures and medications as prescribed to participants
Duties and Responsibilities.
- Seeks and obtains sufficient information necessary to effectively assess participants.
- Performs symptom management based on verbal or physical assessment. When appropriate, offers and completes a clinic or home visit.
- Assess and evaluates the participants status by:
- Initiating orders for medications, durable medical equipment and diagnostics as prescribed by the Primary Care Physician or designee.
- Applying preventative and therapeutic nursing procedures and techniques as appropriate for the participant.
- Initiating or revising the plan of care.
- Conducts on-call/emergent nursing assessments and scheduled visits when assigned and/or indicated to ensure the highest level of physical and emotional support for our participants.
- Communicates with on-site Primary Care Physician (PCP), primary clinic nurse and Charge Registered Nurse (RN) as needed to dispatch hands on care to participant.
- Follow up and routine monitoring of acute and chronic medical conditions following the participants goals of care.
- Additional nursing duties including but not limited to:
- Wound care, Foley care/insertion, IV antibiotics/fluids/TPN, phlebotomy, EKGs, foot/toenail care, medication administration, ear irrigation, and assisting with medical procedures, room participants and dressing/cleaning/transferring participants.
- Works in collaboration with participant/family/caregiver and the Interdisciplinary Team to provide continuity of care and meet the physical and emotional needs of the participant/family/caregiver.
- Must complete all required education and file requirements on time.
- Documents in an accurate and timely manner in the electronic medical record per policy.
- Must care, support, and collaborate with provider and members of the Interdisciplinary Team regarding ongoing patient needs and changes to the plan of care.
- Keeps the leader informed of problems or issues.
- Performs other duties as assigned.
Qualifications
- Bachelor of science degree in nursing preferred; Associate’s degree required.
- RN–Nursing Diploma from accredited School of Nursing with licensure.
- Proficiency with MS Office Suite, Word, Outlook, PowerPoint, and Excel.
- Well-developed ability to show discretion and maintain confidentiality.
- Effective communication skills, both written and verbal.
- Minimum of one year of experience working with the frail, elderly, or long-term care population.
- Minimum of one year of home care experience preferred.
- Minimum of one year of wound care experience preferred.
Specific Requirements
- Possess current CPR certification by American Red Cross or willing and able to obtain as offered by Senior Care Partners P.A.C.E.

Work Environment
- Has a visible presence while working in the center and is subject to frequent interruptions.
- Communicates with staff, participants, caregivers, and other organizations, as necessary.
- Works beyond normal working hours and on weekends and holidays, as necessary.
- May be subject to exposure to TB, AIDS, and/or Hepatitis B virus.
- May work in an unpredictable home environment with variations in cleanliness, temperature, behaviors, smoke, animals, and other potential hazards.
- Subject to callback during emergency conditions (e.g., severe weather, evacuation, disaster, etc.).
- Required to drive a car in all types of weather.
- Fully onsite work environment (clinic and in-home participant care)
Physical Demands (With or without accommodations)
- Must be able to cope with the mental and emotional stress of the position.
- Must function independently, and have flexibility, personal integrity, and the ability to work effectively with participants, staff, and support agencies.
- Must be able to push, pull, move, sit or stand for long periods of time.
- Lift up to twenty pounds as needed.
Minimum Working Requirements
- Must have reliable transportation, current valid Michigan driver’s license and car insurance.
- Must be 18 years of age or older.
- Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B.
Benefits
We offer a competitive benefits package including medical, dental, vision, Teledoc, HelpNet, and company paid life insurance and AD&D at $50,000 each, with the option to purchase additional voluntary insurances – STD, LTD, AI, CI, and additional life and AD&D. Our employees are eligible the first of the month following 30 days of service. All salaried staff receive 20 days of PTO accrued during the year, and non-exempt staff receive 18 days, in which is accrued right away. Employees may start contributing to our 403(b) immediately, and after one year of service the company will match up to 3%. After 90 days of employment full-time employees are eligible for an 8 hour floating holiday, and receive one each calendar year thereafter. All full-time employees receive the six major paid holidays.
