Key Responsibilities:
Deliver skilled nursing care to patients in their homes.
Assess patient needs, develop care plans, and coordinate with interdisciplinary teams.
Educate patients and families on treatment plans, medication management, and overall health.
Maintain accurate patient records and ensure compliance with Medicare guidelines.
Travel between assigned locations as needed to provide care.
