Senior Community Care of Colorado PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare subsidiary of the Volunteers of America parent organization.
Pay: $91-$99k annually
Schedule: Monday - Friday 8AM- 5 PM and participation with afterhours call rotation as needed.

The Clinical Care Manager oversees and manages the overall clinical effectiveness of Senior Community Care (SCC) clinical services to ensure the efficiency, consistency, continuity, and quality of care provided to program participants.
QUALIFICATIONS:
- Bachelor’s or Master’s Degree from an accredited school in nursing strongly preferred.
- Licensed as a Registered Nurse (RN) in the State of Colorado required.
- A minimum of five (5) years of nurse management experience in a hospital, nursing home or community-based setting strongly preferred. Proven experience leading a team of professionals.
- A minimum of one year’s experience in working with the frail or elderly population.
- Must have a valid driver’s license, proof of insurance and have means of transportation.
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Skills and Knowledge:
- Broad knowledge base of physical, mental, and social needs of the frail elderly and their families.
- Skilled in physical assessment and triaging. Ability to react calmly and effectively in emergency situations.
- Skilled in preparing and maintaining medical records.
- Knowledge of PACE regulations, infection control practices, and quality assurance and improvement.
- Strong knowledge about the PACE model and philosophy of care.
ESSENTIAL FUNCTIONS:
- Provides oversight of the overall effectiveness of clinical care provided to SCC participants across all care settings and is responsible to meet defined organizational participant outcome, utilization, and quality goals.
- Provides oversight, management, and coordination of the consistency and quality of the twenty-four (24) hour care provided to program participants across all care settings.
- Provides oversight, management, and direction of the overall operational effectiveness of PACE clinics and is responsible for effectuating and maintaining effective and efficient operational systems, processes, and workflows in compliance with applicable regulations and organizational policies to meet participant care needs and organizational objectives. This includes, but is not limited to, staffing schedules, inventory management and procurement, policies and procedures, clinical workflows, etc.
- Provides direct supervision and support of the Care Managers, Clinic Coordinators, Nursing personnel, Registered Dieticians, Nurse Practitioners, and Rehab Manager.
- Engages in team building. Coaches and mentors staff. Instructs and teaches staff as needed. Trains clinical staff to function independently. Ensures that the job responsibilities, authorities and accountability of direct reports are defined and understood. Assists in the establishment and enforcement of organizational policies and procedures.
- Responsible, in coordination with the clinical team and IDT, for utilization management including minimizing unnecessary or preventable utilization, safe and effective discharges and transitions between care settings, and utilization review and improvement efforts to meet participant care needs.
- In conjunction with the Care Managers, acts as the primary liaison between program and contracted providers and facilities for the coordination of effective care and treatment of program participants.
- Functions as an engaged member of the senior leadership team. In coordination and partnership with other members of the leadership team, assists in the development and implementation of organizational and strategic initiatives.
- Develops, in conjunction with Medical Director, and is responsible for enacting policies and guidelines for the care of program participants to meet quality (including participant satisfaction with services), utilization, and outcome goals including practice guidelines and treatment pathways. Oversees the implementation of the SCC Infection Control program.
- Oversees the SCC Care Management/Care Team Program. Supports the Care Managers in ensuring the participant plan of care is fully developed and accurately reflects participant needs and care and services provided in alignment with organizational policy. Ensures clinical team adheres to the plan of care.
- Oversees and is responsible for the system/process for ensuring both compliance and effectiveness of orientation and ongoing competencies for all staff providing clinical services including RNs, LPNs, CNAs, PCAs, CPhTs, QMAPs, PT, PTA, OT, COTA, and RD. Ensures that clinical staff work effectively within their scope of practice.
- Actively participates and engages in the implementation of quality improvement initiatives. Actively participates as a member of the Quality Assurance Committee.
- Assists in the development and monitoring of quality metrics and indicators. Recommends and implements measures to support ongoing improvement of the quality, consistency, and continuity of participant care and PACE services.
- Establishes and maintains accountability for high customer service standards and monitors, evaluates, and responds to opportunities for improved participant and family/caregiver satisfaction.
- Manages and reviews participant’s records, charts, pertinent information and staff’s documentation. Documents as needed in medical records according to Senior CommUnity Care standards.
- Maintains the stability and reputation of SCC by ensuring all activities and operations are performed in compliance with local, State, and Federal laws, regulations, and contractual requirements.
- Establishes and maintains a productive working relationship with relevant contracted providers.
- Engages in team building. Coaches and mentor staff. Instructs and teaches staff as needed. Trains clinical staff to function independently. - Is responsible for adherence to program cultural standards (Fundamentals) including supporting the development and integration of these standards within the team through modeling, coaching, and accountability.
- Follows and ensures that clinical team follows all organizational policies and procedures and CDC and Occupational Safety and Health Administration (OSHA) safety guidelines.
- Educates staff in regard to care and treatment for the elderly as needed.
- Acts within the scope of his or her authority to practice.
- Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.
- Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
- Performs other duties as required or requested.
