- Develop and implement effective plans of care for participants, working closely with the IDT to ensure seamless care coordination.
- Provide direct supervision to home care staff, including scheduling management and performance evaluations.
- Ensure compliance with regulatory requirements, including OSHA safety guidelines and Senior CommUnity Care policies and procedures.
- Maintain accurate records and reports, including participant medical records and departmental statistics.
