Senior Community Care of Colorado is a PACE Program serving older adults in Montrose and Delta Counties. Our team offers wrap-around care services to meet the unique needs of each of our participants, supporting them to remain living independently in their homes and communities.
We are seeking a skilled Nurse Manager for Home Care to oversee the day-to-day operations and operational effectiveness of the Home Care department. The successful candidate will be responsible for providing direct supervision to home care Personal Care Attendants and C.N.A.s, as well as ensuring the overall quality of care delivery.
This role requires strong leadership and organizational skills, with the ability to prioritize tasks effectively and manage competing demands. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work collaboratively with multidisciplinary teams to achieve high-quality outcomes.

Key Responsibilities:
- Provide direct supervision to home care staff, including scheduling management and performance evaluations.
- Develop and implement effective plans of care for participants, working closely with the Interdisciplinary Team (IDT) to ensure seamless care coordination.
- Ensure compliance with regulatory requirements, including OSHA safety guidelines and Senior CommUnity Care policies and procedures.
- Maintain accurate records and reports, including participant medical records and departmental statistics.
