Senior Community Care of Colorado (SCCCO) is a PACE Program serving older adults in Montrose and Delta Counties. As a Home Care Coordinator LPN, you will play a vital role in delivering high-quality care services to our participants.
">About the Role:
">This role requires a strong understanding of health care problems of the frail elderly, medical equipment, and instruments. The ideal candidate will have experience in nurse management, preferably in a hospital or community-based setting.
">Responsibilities:
">- Oversight and supervision of all home care aides and home care scheduler/lead C.N.A.
- Schedule management and overall operational effectiveness of the department.
- Performing initial, semi-annual & change of status assessments of participants in their home environment.
- Coordinating plan of care with care-giving resources and ensuring homecare staff follow the participant's plan of care as approved by IDT.
Requirements:
">- Licensed Practical Nurse (LPN) in the state of Colorado
- Minimum of two (2) year's nurse management experience in a hospital, nursing home or community-based setting
- Home health experience preferred
