Location : Visits across Inland Empire
Schedule : Full-Time / Part-Time
Compensation : $30-$35 / hour, commensurate with experience
Position Summary :
We are seeking a compassionate and skilled LVN to join our Palliative Care Program. In this role, you will provide hands-on nursing care and emotional support to patients in their homes, assisted living facilities, or skilled nursing facilities. Your role is vital in ensuring patients receive the highest quality care with dignity and respect.
Key Responsibilities :
- Provide direct patient care, including medication administration, wound care, and symptom management, under the direction of a Registered Nurse.
- Support patients and families by offering education on symptom management, coping strategies, and palliative care resources.
- Collaborate with the interdisciplinary palliative care team to implement personalized care plans.
- Assist patients with activities of daily living, promoting comfort and independence wherever possible.
- Maintain accurate and timely documentation of patient care and assessments.
- Serve as a compassionate resource for patients and families, offering emotional support and guidance.
- Participate in legacy work and memory-making activities to help families find meaningful closure.
