Senior Doc is expanding in the Los Angeles, CA area [Boyle Heights, West Hollywood, Hollywood, Alhambra] , and we're on the lookout for a Certified Nurse Practitioner or Physician Assistant with a passion for serving the senior population.

What You'll Do:
Provide in-home primary care visits for seniors in private homes, assisted living, and skilled nursing facilities.
Complete comprehensive assessments, diagnose and manage chronic conditions, and develop personalized care plans.
Collaborate with care teams to deliver holistic, patient-centered care.
Build trust and rapport with patients and their families through strong communication and a warm, compassionate approach.
Qualifications:
Active and unrestricted NP or PA license in California.
Experience in geriatrics, primary care, or home-based care preferred.
A heart for seniors and a drive to provide accessible, relationship-based care.
