What are the responsibilities and job description for the LPN Home Health Nurse-30 Days Out position at Senior Friend Home Care?
The Home Health LPN is responsible for providing home health services and care to patients and their families at their homes. The LPN will work under the supervision of a registered nurse. All patient visits are conducted at their homes. The LPN is responsible for identifying patient needs through continuum of care and quality assurance in coordinance with the RN, DON, and clinical team. The Home Health LPN is responsible for in person visits while ensuring patients quality of care, safety, and best care outcomes.

Pay: $28.00 - $32.00 per hour
