We are expanding and in need of Care Team Coordinators to work in the Western suburbs in and around New Lennox.
This full time position is ideal for a Caregiver with a least 2 years experience. Experience must be on the job and not personal care for family.
The ideal candidate will have experience with personal care with both mobile and bed bound clients.
The roll does involve being in the field supporting our clients as needed but ALSO includes working in the office learning administrative skills and supporting the office team as needed.

Duties Include:

Coordinating the caregivers, including support with hiring, first day and ongoing training, performance improvement, and implementation of caregiver training programs in collaboration with the supervisor

Conduct quality assurance site checks of each caregiver to ensure care plans are being implemented correctly, policies and procedures are adhered to, and all job duties are performed in a manner that is satisfactory and meets the Senior Helpers brand expectations.

Reviews the daily logs and timesheets to ensure that each caregiver is following the care plan and that lost hours are identified.

Supports with new hires, terminations, and inactive employees communicating to the Scheduling Manager.

Cover Caregiver call offs on a daily or weekly basis as needed either with helping to find Caregiver replacement or going in person to cover shifts and assist our valued clients ensuring they do not get left without care.

Collaborate with Human Resources with relation to coaching and training of internal staff members; ensure staff communications are accurate and thorough.

Adhere to federal, state, and local laws and regulations.
Qualification Required:

CNA an advantage

Must have minimum of 2 year experience as a Caregiver including having completed personal care, companionship and support to clients with dementia

Must have excellent organizational skills, attention to detail, and the ability to prioritize in a changing environment.

Ability to use critical thinking to proactively prevent and solve problems.

Ability to visit clients’ homes in the assigned territory.

Ability to learn software programs quickly.

Strong communication skills

Ability to work independently and as part of a team.
