Title : Clinical Program Director
Position Description : Full-time, day shift (Monday- Friday)
Pay Range : 80,000-$85,000 Salary
Overview :
The Clinical Program Director plays a crucial leadership role within a home care agency, responsible for overseeing the services provided to clients, managing caregivers and staff, and ensuring the highest level of care quality and client satisfaction. This role is instrumental in maintaining the agency's standards of excellence and promoting the well-being of clients.
Leadership Responsibilities :
- Developing patient plans of care, managing changes to patient plans of care and instruction to care staff.
- Training staff (See below).
- Developing, instructing and managing time reporting systems / mechanisms for staff.
- Performance of clinical staff evaluations and competency determinations.
- Establish and monitor key performance indicators (KPIs) to assess care quality and client satisfaction.
- Implement continuous improvement initiatives based on feedback and data analysis.
- Foster relationships with healthcare providers, community organizations, and referral sources to expand the agency's client base.
Clinical & Care Management :
Complete clinical assessments for all Medicaid clients in concordance with state and federal regulations.Experience making referrals for patient services.Obtaining written orders from physicians consistently and effectively.Ensure compliance with all relevant state and federal regulations, including licensing and certification requirements.Maintain accurate client records and documentation.Ensure the delivery of high-quality care services that meet the physical, emotional, and social needs of clients.Adjust care plans as necessary to address changing needs or conditions.Act as an advocate for clients and their families, addressing concerns, requests, and feedback.Serve as a liaison between clients and the agency.Participate in community events, seminars, and health fairs to promote agency services.Training Responsibilities :
Function as a resource and mentor to peers and subordinates to maintain professional nursing standards.Train and supervise caregivers, home health aides, and nursing staff.Provide ongoing guidance, supervision, and performance evaluations for caregivers.Develop and provide ongoing training and development programs for caregivers and staff.Evaluate skills training and sign off on all completed training.Conduct initial and periodic assessments of client needs to create personalized care plans.Coach all staff in proper safety procedures and consistently reinforce protocols.Keep staff informed of best practices, safety protocols, and care techniques.Develop and implement emergency response plans for clients, ensuring the safety and well-being of clients during emergencies.Qualifications :
Bachelor's degree in Nursing, Healthcare Administration, or a related field (Master's degree preferred).Proven experience in a leadership role within the home care or healthcare industry, with a minimum of 5-7 years of relevant experience.Must have experience in provision of personal care services or in a related program, including but not limited to; HHA, Hospice or Community Based Residential Facility.Strong knowledge of home care regulations and best practices.Excellent communication, interpersonal, and leadership skills.Exceptional organizational and problem-solving abilities.Compassion, empathy, and a client-centered approach to care.Proficiency in healthcare management software and electronic health records.Ability to work under pressure and make critical decisions.Knowledge of medical conditions, treatments, and care techniques.Experience making referrals for patient services as well as obtaining and understanding written orders from physicians.An Equal Opportunity Employer.
