Care Coordinator

This position is FULL TIME and hours are Monday thru Friday 9am-5pm. This position will be based out of our Warren, PA, office. Travel is required. If any of the following interests you – APPLY! We would love to chat with you – to see if you might be the right fit for our team!

Requirements:

• Previous experience in Home Care/Health Care, Social Services, Case Management preferred.
• Experience completing level-of-need assessments preferred.
• Previous experience with Dementia, Parkinson’s, Mental Health, Chronic Pain is a plus.
• Must have reliable transportation, valid driver’s license, and auto insurance.
• Must be willing to travel throughout multiple counties in our region.
• Communication & Computer Skills a must.

Essential Duties & Responsibilities:

· Travel between our Warren Office and multiple counties in our Region to perform in-person client assessments in participants’ homes.

· Create Care Plans for new participants and update Care Plans of existing participants.

· Be responsible for data collection and entry on multiple platforms.

· Perform pop-in visits, reassessments, caregiver and client “Meet & Greets”.

· Present orientation and skills training for new hires.

· Assist caregivers with general employment questions and client care concerns.

· Demonstrate strong communication skills and develop rapport with both clients and caregivers.

· Work well independently and with multiple departments within the company.

· Abide by regulatory compliance, local, state, and federal laws – including but not limited to – HIPPA.

· Future On-Call Rotation will be expected.

Pay: $18.00 - $20.00 per hour
