FVK is recruiting for a Managed Care/Discharge Coordinator to join our team! Starting pay is $19.00 plus years of experience. This position is responsible for working with new admissions for the initial and on-going insurance authorization for services for Managed Care. Assures all updated information is presented to the insurance company as required. Discharge planning of rehab residents by initiating admission welcome meeting and goal setting for discharge. Initiating the discharge process for the other disciplines and assuring all discharge needs are met.

Here are a few of the daily responsibilities of a Discharge Coordinator
- Verify insurance coverage at time of admission and at the start of rehab services.
- Communicate insurance coverage appropriately to the interdisciplinary team including coverage criteria, rates, and approval dates.
- Maintain appropriate contact schedules for pre-authorizations, re-authorizations, and appeals.
- Two or more years of experience working with insurance coverages; preferably in a long-term care setting
- Excellent communication skills, both written and oral; with the ability to influence and engage a wide range of donors and build long-term relationships.
- Team player who will productively engage with others on the interdisciplinary team
- Strong computer skills including Microsoft Office, Rehab Optima and Point Click Care
- Understanding of Medicare and Managed Care Coverage Criteria for care services
