Clinical Manager

Position Summary:

We are seeking a skilled and compassionate Clinical Manager to oversee the delivery of high-quality, patient-centered care in our Medicare-certified home health program. This role is responsible for supervising clinical staff, ensuring regulatory compliance, and supporting positive patient outcomes across the home health continuum.

Key Responsibilities:

• Lead, manage, and support a team of home health clinicians including RNs, therapists, and aides.

• Ensure all clinical care meets Medicare, state, and agency standards.

• Review and monitor documentation, OASIS accuracy, and care coordination.

• Oversee case conferences, clinical audits, and quality improvement activities.

• Facilitate collaboration among interdisciplinary team members and external providers.

• Manage clinician schedules, productivity expectations, and staff development.

• Ensure clinical readiness for accreditation and regulatory surveys.

• Promote a culture of excellence, compliance, and compassion in care delivery.

Qualifications

Qualifications:

• Active RN license in [State] (required).

• BSN required; MSN or other advanced degree preferred.

• Minimum of 3 years' experience in Medicare-certified home health.

• At least 2 years of clinical leadership or management experience.

• Strong working knowledge of Medicare Conditions of Participation (CoPs).

• Proficiency with EMR systems and understanding of clinical documentation standards.

• Excellent interpersonal, leadership, and critical-thinking skills.

Preferred Qualifications:

• Experience with OASIS documentation and QAPI initiatives.

• Bilingual abilities a plus.
