Position Summary:
We are seeking a skilled and compassionate Clinical Manager to oversee the delivery of high-quality, patient-centered care in our Medicare-certified home health program. This role is responsible for supervising clinical staff, ensuring regulatory compliance, and supporting positive patient outcomes across the home health continuum.
Key Responsibilities:

Lead, manage, and support a team of home health clinicians including RNs, therapists, and aides.

Ensure all clinical care meets Medicare, state, and agency standards.

Review and monitor documentation, OASIS accuracy, and care coordination.

Oversee case conferences, clinical audits, and quality improvement activities.

Facilitate collaboration among interdisciplinary team members and external providers.

Manage clinician schedules, productivity expectations, and staff development.

Ensure clinical readiness for accreditation and regulatory surveys.

Promote a culture of excellence, compliance, and compassion in care delivery.
Qualifications
Qualifications:

Active RN license in [State] (required).

BSN required; MSN or other advanced degree preferred.

Minimum of 3 years' experience in Medicare-certified home health.

At least 2 years of clinical leadership or management experience.

Strong working knowledge of Medicare Conditions of Participation (CoPs).
