Job Title: Licensed Practical Nurse (LPN) - Per Diem
Job Summary:
We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) for per diem home health positions. The ideal candidates will provide high-quality nursing care to patients in their homes, ensuring adherence to Medicare home care standards and company policies.
Key Responsibilities:

Provide skilled nursing care under the supervision of an RN.

Administer medications, wound care, and other nursing treatments as prescribed.

Monitor patient conditions and report changes to the healthcare team.

Educate patients and families on care plans, medications, and disease management.

Maintain accurate and timely documentation in compliance with Medicare guidelines.

Travel between assigned locations to deliver in-home care.
