Job Summary:
SN Homehealthcare is seeking an experienced and compassionate Registered Nurse (RN) to provide comprehensive, patient-centered care in the comfort of patients’ homes. As an RN, you will assess, plan, and deliver individualized care to a diverse patient population while coordinating with interdisciplinary team members. The ideal candidate is self-motivated, organized, and committed to improving the health and well-being of patients.
Key Responsibilities:
Conduct initial and ongoing patient assessments to determine health status and care needs.
Develop, implement, and regularly update individualized care plans in collaboration with the patient, their family, and the care team.
Provide skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.
Educate patients and their families on health conditions, treatment plans, medication use, and preventive care strategies.
Supervise and mentor Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) to ensure quality care delivery.
Coordinate with physicians, therapists, social workers, and other healthcare professionals to ensure seamless care transitions and effective treatment outcomes.
Monitor patient progress, document observations, and communicate any changes in condition to the care team promptly.
Ensure compliance with agency policies, state and federal regulations, and accrediting standards.
Utilize electronic medical records (EMR) to maintain accurate and timely documentation.
Participate in case conferences, quality improvement initiatives, and ongoing training
Job Qualifications
Qualifications:
Current and valid Registered Nurse (RN) license in [State].
Bachelor’s Degree in Nursing (BSN) preferred but not required.
Minimum of 2 years of clinical nursing experience, preferably in home health, acute care, or community health.
Current CPR and First Aid certification.
Proficiency in clinical skills, including assessment, care planning, and critical thinking.
Strong interpersonal and communication skills for patient and family education.
Ability to work independently, manage a flexible schedule, and make decisions in home-based settings.
Reliable transportation and a valid driver’s license.
Experience with electronic medical records (EMR) systems is a plus.
