TITLE: Social Service Director, CSW
REPORTS TO: Administrator
SUMMARY:
Under the direction of the Administrator, the Social Service Director shall plan and organize the social service department of the Adult Care Program as well as utilize appropriate community agencies that can provide economic, psychological, and medical and other support services. The qualifed person should be have strong interpersonal skills as well as the ability to work and function independently in a fast pased enviorment. Coordination skills are vital in this field, to work with with nursing departments and HMO Insurances and social service agencies.

REQUIREMENTS & EDUCATION:
- Bachlors Degree
- New Jersey License in Social Work or Clinical Social Work
- New Jersey Certification in Social Work with a minimum of 1 year's social work experience in a health care facility..
- Provide social care management to the Center's Members.
- Provide a liaison between Member/staff, Member/agencies and Member/family.
- Social Service Assessment.
- Home/Living Assessment and complete mandated documentation.
- Interdisciplinary Care Plan (admission/quarterly review/annually).
- Summaries (quarterly), Event Triggered Notes (occurrence).
- Quality Assurance quarterly
- Medicaid Profiles (quarterly).
- Participate in in-service education.
- Provide social service information to the Members and staff as needed.
- Make home visit is necessary.
