Title: Case Management Community Liaison
Job Classification: Exempt, Full-time (40 hours)
Salary Range: $40-$45K per year, based on experience
Location: Onsite, SENT, in the Hi-Crest area
Position mission and Major Duties:
SENT is looking for a Case Management centered Community Liaison, that can screen and evaluate community members for needs of service. This role will meet with community members, build a network of partners/resources, and collaborate with fellow SENT team members to support SENT programming. This role will supervise and train team members, volunteers, and vendors on program protocol and safety compliance.

Position Qualifications:
The ideal candidate will have, or be willing to, obtain a Person-Centered Case Management (PCCM) certification. The candidate will have a willingness to expand knowledge of community resources and develop appropriate connections – as well as – be a good advocate. The role has a high level of confidentiality, as required by HIPAA. SENT needs a tech savvy individual with advanced Microsoft Office Suite skills and a familiarity with G Suite/Google Drive. (must have a valid Kansas driver’s license)
