Sentara Health is seeking a dedicated Integrated Nurse Case Manager/RN/OBGYN - Western Tidewater region of Virginia!
This full-time, work-from-home position requires travel to conduct face-to-face home visits in the member’s home within the Western Tidewater region of VA.
This includes but is not limited to the following areas: Franklin, Suffolk, Isle of Wight County (Smithfield, Windsor), and Southampton County (Courtland, Capron, Ivor).
Applicants must reside in or near one of these locations to be considered for the role.
Education
- BSN (Bachelor of Science in Nursing) preferred
Certification/Licensure
- Registered Nurse License (RN) - Nursing License - Compact/Multi-State License require
Experience
- Nursing experience, with a focus on case management, care coordination, or patient advocacy, Medical/Clinical cases for pregnant and postpartum women.
- Maternity experience (L&D, Postpartum, Mother/Baby) is preferred.Maternity experience (L&D, Postpartum, Mother/Baby) is preferred.
- Strong knowledge of physical, psychological, socio-cultural, and cognitive patient needs.
- Excellent communication skills, both oral and written, as well as strong problem-solving and analytical skills.
- Ability to work independently and collaboratively in a remote, fast-paced environment.
