Summary
The CFS Specialist is responsible for providing personalized support and resources to individuals with developmental disabilities and their families. This role is focused on helping clients live successfully within their family homes and communities, guiding them through the process of accessing essential services and support systems. The CFS Specialist will work collaboratively with clients, families, and community partners to promote independence, resilience, and quality of life.
Key Responsibilities:
Family Home Support
- Help clients thrive in their family homes by implementing tailored support strategies.
- Assist families in creating structured routines, schedules, and home adaptations.
Service Coordination & Navigation
- Guide families through complex service systems, including Regional Center resources and public benefits.
- Identify and overcome barriers to accessing essential support services.
Education & Resource Provision
- Provide families with information about diagnoses and available resources.
- Offer training sessions and materials to enhance caregiving skills.
Cross-Provider Coordination
- Ensure consistency in care by facilitating communication across service providers.
- Schedule and manage medical and therapy appointments for comprehensive care.
Transportation & Scheduling
- Arrange transportation for clients to appointments and community activities.
- Maintain flexible scheduling to meet client needs.
Crisis & Backup Support
- Develop emergency backup plans for service disruptions.
- Provide or arrange temporary direct support during urgent situations.
Future & Transition Planning
- Assist clients in planning for independent living, employment, and long-term care.
- Support families in preparing for caregiving transitions, especially for aging caregivers.
Skills Training & Community Engagement
- Teach independent living and safety skills to clients.
- Organize community activities that promote social inclusion and personal development.
Administrative Duties
- Maintain accurate documentation of service plans and client progress.
- Submit quarterly reports in compliance with regulatory standards.
Qualifications:
- Education & Experience:
- Associate’s degree in human services or related field (Bachelor’s preferred).

- Language & Communication:
- Ability to communicate in the client’s primary language (e.g., English, Spanish, Mandarin).
- Strong written and verbal communication skills for clear documentation and reports.
- Technical Skills:
- Experience with scheduling software and case management tools.
- Familiarity with community resources and assistive technologies.
- Certifications & Clearances:
- Valid driver’s license and reliable transportation.
- Ability to pass a background check per California regulations.
- First Aid and CPR certification.
