Sequoia Transitional Care is seeking a skilled and compassionate Registered Nurse (RN) to join our nursing team as a PM Floor Nurse. This is a full-time, weekday position with some flexibility, ideal for a nurse who thrives in a collaborative, resident-focused environment.
We’re offering competitive pay and a $1,500 sign-on bonus for candidates who are ready to make a difference in long-term and transitional care.
️ Your Responsibilities:
Provide direct nursing care to residents in accordance with individualized care plans and physician orders
Administer medications and treatments accurately and on time
Monitor and report changes in resident health or behavior to appropriate team members
Coordinate with CNAs and interdisciplinary staff to ensure consistent, quality care
Maintain accurate documentation using facility EMR (PointClickCare preferred)
Assist with admissions, discharges, and transitions of care
Respond to emergencies and follow established protocols
Uphold infection control and safety policies
Qualifications:
Current and active RN license (California)
CPR certification (required)
Experience in a skilled nursing or long-term care facility preferred
Strong clinical judgment, communication, and leadership skills
EMR experience (PointClickCare is a plus)
Compassionate and resident-focused mindset
