PATIENT CARE COORDINATOR
Serene Home Nursing Agency is looking for a full-time Patient Care Coordinator to join our team!
*This is NOT a remote position! Only applicants that reside on Long Island will be considered!!
The ideal candidate must have:
· Prior staffing experience required- healthcare staffing experience highly preferred!!
· Proficient computer skills
· Experience working with patients and their families
· Must have professional phone etiquette and problem-solving strategies
· Compassionate, with excellent attention to detail
· The ability to collect and maintain information in our scheduling software program with precision and detail
· Electronic shift verification and updating experience
· The ability to work as a team member with the other staffing coordinators and other agency staff to affectively provide services to patients and facilities in the community
· Must be able to handle a high call volume and multitask
· The candidate must be punctual, understanding, and professional in the office and in daily communications with patients and their families.
· Must be able to handle difficult situations involving customer concerns in a professional and empathetic manner.
· Ability to be on-call approximately 2 times a week after hours for patient issues
A plus if the candidate has:
· Experience with HHA Exchange
· Experience with ALAYACARE
· Bilingual
The daily routine will include:
· Maintain a caseload of 35-50 patients
· Finding the appropriate staff to fill home care shifts for our patients in need of services to assure case coverage is met
· Document employee absences and fill open shifts.
· Possess the ability to form relationships with field staff and recognize trends to be proactive for upcoming case staffing needs.
· Maintaining and updating patient scheduling records timely and accurately.
· Collecting and maintaining information in our scheduling software.
· Updating patient and caregiver profiles and schedules in real time.
· Work closely with the nursing supervisory department and other medical professionals for patient medical-related issues when needed.

**PAY: $22-$26 per hour BASED ON EXPERIENCE
