POSITION OVERVIEW: This position is based at a small facility licensed for up to 30 residents, fostering a close-knit and personalized care environment.The LVN Charge Nurse is responsible for overseeing the nursing care of residents during their shift and serves as the top on-duty supervisor when management is out of the building. An administrative staff member will be available on-call for emergencies.
QUALIFICATIONS AND REQUIREMENTS
Education & Certification:
- High school diploma or equivalent.
- Current LVN license issued by the State of Texas.
- Current CPR certification.
- TB skin test or Chest X-Ray and up-to-date immunizations as recommended for adults by the Health Department.
Experience:
- 3–5 years of geriatric nursing experience.
Skills & Attributes:
- Strong physical and mental health, with the ability to communicate effectively.
- Ability to follow directions from the Administrator and takes charge when necessary.
- A genuine interest in geriatric nursing, with a patient and compassionate demeanor toward residents.
- Professional appearance: neat, well-groomed, and in uniform.
- Dependable and responsible.
RESPONSIBILITIES
The LVN Charge Nurse will perform duties that include, but are not limited to:
Resident Care:
- Providing nursing care to residents, following established routines and specific instructions from the Administrator.
- Administer medications and treatments as prescribed, ensuring proper documentation in the Medication Administration Record (MAR).
- Monitor residents' health, promptly reporting any unusual symptoms to the physician or Administrator.
- Conduct and document assessments, including skin, elopement risk, fall risk, and other required evaluations.
Supervision & Documentation:
- Oversee personnel working in the nursing department during their shift.
- Complete necessary paperwork accurately and in a timely manner.
- Record incidents or accidents on appropriate forms, notify family and physicians as needed, and inform both the Administrator and the state of Texas.
Communication & Emergency Response:
- Act professionally when answering phones, providing information, and taking messages during unmanned office hours.
- Handle emergencies effectively, ensuring proper reporting and documentation of incidents after the situation is under control.
Administrative Duties:
- LVN must possess proficiency in computer skills.
- Maintain awareness of each resident’s service plan and goals, striving to support and further those objectives.
- Attend mandatory in-service training and complete at least eight (8) hours of continuing education annually, as required by state regulations.
- Participate in fire drills and be familiar with Serenity Assisted Care Living’s Disaster and Emergency Plan, including implementation procedures.
Professional Conduct:
- Maintain confidentiality regarding residents’ personal information, business activities, and interactions at Serenity Assisted Care Living.
- Foster positive relationships with the team, residents’ family members, and other visitors.
- Ensure a proper handoff at the end of each shift, including giving a detailed report to the next charge nurse before leaving the facility.

- Perform additional duties as assigned by the Administrator.
