The Program Coordinator position is responsible for ensuring the development, implementation, coordination, and monitoring of ISP's, Behavioral and/or Therapeutic Treatment Plans for individuals on their caseload within the Sponsor program.
Responsibilities:
- Perform monthly home inspections/visits of Sponsored Residential Homes
- Document and create reports (incident reports, annuals, etc.)
- Coordinate and conduct various meetings (intakes)
- Maintain current and accurate files on caseload (collect and review monthly documentation)
- Develop and maintain relationships with individual's family members/guardian(s), social worker's, CSB's, etc.
- Assess individual needs for recommendations and input (verbal/written) regarding issues of admission, transfers, and discharges
- Ensure adherence to licensure (Human Rights, State/Federal/Local Regulations)
- Review support data and progress notes (support plans, etc.)
Qualifications
- 1 year of experience providing direct services with adults diagnosed of an intellectual disability or other developmental disabilities
- 1 year experience interpreting and implementing federal, state, or local regulations (DBHDS)
- Bachelor's degree in the Human Service's field (Sociology, Social Work, Special Education, Psychology, etc.)
- Experience with Therap's (preferred)
- Experience with WAMS (preferred)
- Excellent verbal and written communication
- Ability to work closely with clients
- Interpersonal skills
- In-depth knowledge of incident reporting
- An understanding of DBHDS and DMAS
Job Type: Full-time
Pay: $41,000.00 - $50,000.00 per year
